or
He is survived by
and why?

"He was survived by" suggests, in a narrow sense, that the people named were alive at the moment of his death. "He is survived by," the likelier construction, suggests that the people named are still alive at the time of publication.

I'm guessing he is survived ... Though he may be in the past tense, by definition those who survive are in the present.
